WellCheck Joins St. Mary’s 360 Initiative to Expand Connected Care Across St. Mary’s County

Health tech company will support the $3.5 million investment to strengthen digital connections to health and community services across St. Mary’s County

MONKTON, MD, UNITED STATES, September 10, 2026 /EINPresswire.com/ — WellCheck, a Maryland-based public health technology company, today announced its participation as a project partner in St. Mary’s 360: Connected Care Everywhere, a new countywide initiative led by the St. Mary’s County Health Department (SMCHD).

SMCHD has been awarded $3,530,907 through the Maryland Rural Health Transformation Program to launch the initiative, which aims to improve access to care and health outcomes for St. Mary’s County residents.

St. Mary’s 360 will bring together local organizations, trusted services, and technology to help residents connect with healthcare, behavioral health, oral health, wellness programs, digital health support, and other community resources. The initiative is designed to strengthen connections among residents, healthcare providers, public health programs, and community partners.

A central component of St. Mary’s 360 will be Community-Integrated Care Coordination, which brings care coordination into the clinics, schools, community programs, and other trusted settings where residents already live, learn, work, and receive care. Supported by WellCheck’s digital infrastructure, this approach will help residents navigate services, complete referrals, and remain connected to ongoing care. The program will also support digital health literacy by helping residents build the skills and confidence needed to use the Health & Wellness Portal, telehealth services, remote-monitoring tools, and other digital health resources.

Under St. Mary’s 360, the Health & Wellness Portal will expand as a local directory, referral, and care-coordination resource. The broader initiative will also include:

Community-integrated care coordination: Dedicated care coordinators will work across clinics, schools, community programs, and other local access points, using connected digital tools to help residents navigate services, complete referrals, and stay engaged with ongoing care and support.
*Assistance with telehealth, remote monitoring tools, and eligible home health-monitoring devices;
*Mobile, home-based, and community-based outreach for residents who experience barriers to care;
*Stronger connections to primary care, chronic-disease prevention, behavioral health, oral health, and wellness services; and
*Improved coordination among healthcare providers, public health programs, and community organizations.

In addition to WellCheck, project partners and supporting organizations include Greater Baden Medical Services, the St. Mary’s County Ambulance and Rescue Squad Association, the St. Mary’s County Department of Emergency Services, the St. Mary’s County Detention and Rehabilitation Center, the St. Mary’s Family YMCA, and the University of Maryland Horowitz Center for Health Literacy.

The initiative builds on existing services and partnerships to create a more connected local system—one that reaches residents earlier, supports them longer, and helps them successfully navigate the path to better health.

The Maryland Rural Health Transformation Program is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award to the Maryland Department of Health totaling $168,180,837.61 with 100 percent funded by CMS/HHS.
